Western Tidewater Free Clinic Eligibility Requirements
Eligibility Screening
The Western Tidewater Free Clinic does eligibility screenings by appointment only. Please call 757.923.1060 to be scheduled. We serve residents of the cities of Franklin and Suffolk, and the counties of Southampton and Isle of Wight. You MUST bring the following items when you come to the eligibility screening:
(1) PHOTO IDENTIFICATION CARD
(2) PROOF OF INCOME FROM EVERYONE IN THE HOUSEHOLD WHO CURRENTLY HAS INCOME FROM ANY SOURCE, INCLUDING:
- Past 2 consecutive pay stubs
- Food stamp verification letter
- Unemployment letter (from VEC)
- Disability letter
- Proof of child support / alimony
- Federal Tax Return
3) PROOF OF SOCIAL SECURITY NUMBER
4) PROOF OF LEGAL ADDRESS, such as one of the following:
- Voter ID card
- Car insurance
- Utility bill
- Lease/rent agreement
- Letter from Homeless Shelter or Social Services regarding your current living situation
IMPORTANT: You must have ALL FOUR (4) of these items to be considered eligible for services. If you are found to be eligible you will be given an appointment for an upcoming clinic.
